Patient Account Setup (Web Browser)

In order to use the Hale Health application, you'll need to perform the following steps to get your account created and verified. These instructions will walk you through activating from a web browser.

Open a web browser (Chrome preferred) and navigate to https://app.hale.co/activate 

Enter your First Name, Last Name, Birthdate, and Last 4 of your SSN, and click Next:

It’s important to note that the first and last name must match your patient record exactly, nicknames may sometimes cause trouble. Please contact your clinic directly to confirm how your first and last names are reflected in your patient record if run into any issues on this step.

Verify your identity by receiving an access code:

If you're not receiving the access code via text when selecting your phone number, please contact your clinic directly to obtain an access code over the phone. You can then select "I already have an access code" and continue the registration process.

Enter your code and click Verify (example access code):

Specify your email address and password for the account, click Next:

You've completed the registration and activation process.

For continued access, you can browse to https://app.hale.co and login with your credentials.
